This services are carried out at our premises by appointment only.

district nurses are based at Waldron health centre and can be contacted directly on 020-3049 3400 or you may be referred by your GP The
Health visitor may be contacted on 7771 5734.
SICK NOTES
We do not issue NHS medical certificates for a period of illness lasting less than 7 days. Self certification forms, available from reception, should be sufficient for your employer. If you still require a private medical certificate you will be charged for this, in line with the current fees recommend by the BMA. A list of fees is available in reception.
