Patient Registration
You may register with us as an NHS patient if you are normally a resident in the area shown on the back of this leaflet. You should have a health check with the nurse, so that we may have as much information about your health as possible. We need to have all the basic information about you on file should you need a report or a form for your insurance company or your employer. We cannot prescribe medication for you until you have seen a doctor.
Pre-registration
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Overseas And Temporary Visitors
If you are an overseas visitor you may be asked to pay for your treatment unless you are from an EEA member country carrying a form E112 or a form E128. As a visitor the doctor may provide you with immediately necessary treatment free of charge in an emergency.
If you are temporarily resident in the area and not eligible for free NHS treatment, the doctor may accept you as a private patient. If you are temporarily resident in the area for a period of up to 3 months, and eligible for NHS treatment, we may accept you as a temporary patient.
Your medical records remain with your registered GP but we will provide you with full medical care while you are in the area. If you leave the UK for a period of more than three months the doctor is no longer responsible for your medical care and may not provide repeat medication in your absence.
